VIII National Congress of Health Care for Chronic Patients –

He VIII National Congress of Health Care for Chronic Patients and the III National Conference of the Active Patient will take place in the Municipal Palace of Congresses of Madrid on April 7 and 8.
Promoted by the Spanish Society of Internal Medicine () and the Spanish Society of Family and Community Medicine (), nOnce again, this forum will be a perfect space to discuss care models for chronic patients and the challenges that chronic diseases pose to the healthcare system. Thus, in this Congress the advances in chronicity care will be announced, experiences will be shared and the best practices in this field will be analyzed from a multidisciplinary and intersectoral vision, with the aim of promoting proposals and alternatives for change in our health and social services system.

ADVANCING IN STRATEGIES AND ACTIVE RESULTS: THE DEPLOYMENT OF CHRONICITY MODELS There will be an initial presentation on the progress of the Strategy for the approach to chronicity in the National Health System (20 minutes). Next, another speaker in a 15-minute presentation will present the results of an audit whose objective was the Benchmark between different autonomous communities and which was carried out in 13 of them on the development and level of implementation of elements related to the chronicity strategies of the Autonomous Communities in 2015. Subsequently, the referents of the Autonomous Communities with the best results in some block will be invited to briefly present their actions in those elements in which they have shown the best results and ask them questions about these aspects. They will be representatives of the autonomous communities of Madrid, Castilla y León, Catalonia, the Valencian Community, Galicia and the Basque Country.
