Keeping paper medical records updated and organized is not my strong suit. So, instead of fighting against the tide, I decided to prepare a digital system that would allow me to keep all of my patient information organized and easy to review and maintain. I have used this template for the last few months with very good results and I think it is a good time to share it with you. I hope you find it useful.
The template is created in Notion, a free and very powerful notes application that allows you to organize any type of information. With this template you can add all the information about your patients (from basic data, to the psychiatric medication used, emergency contacts, clinical notes for each session and you can even add resources, links, PDFs and guides that you want to use with each patient.
You can add the general data of your patients. Different categories of information are displayed according to your needs. Clinical notes of each session. You can add text, tables, images and everything you need. You can add any type of resources: PDF, manuals, images, audios, etc.
How to use the template
To download the template you must and then click on duplicate. With that you can use it from the application Notion on your cell phone or computer.
The template has several levels of complexity and you can use them according to your needs. On the first page you will find a very simple table that shows the patient’s name, zoom link (in case you attend online) and dates of the next session. This will allow you to quickly review the work ahead of you. Clicking on the patient’s name will open another page with general information (name, birthday, age, address, etc.). Then you can click on the icon Clinical notes and the data will be opened with information on medications, reason for consultation and treatment plan and clinical resources.
